In most cases a physician will provide a referral to begin treatment. However, in New York State patients can be seen via Direct Access without a prescription. Please see below for more information on Direct Access.

All new patients need to bring their physical therapy referral and insurance card to their first appointment. Intake forms are available for download at the bottom of this page.

Initial evaluation typically lasts one hour, and subsequent appointments are between 30 minutes and 60 minutes depending on the condition. Your therapist will be able to provide you with a more accurate time.

In most cases, you can start physical therapy and get the relief you need right now without a physician prescription. You do not need a referral from a physician to begin therapy at Altamont PT. You have “direct access” to physical therapy for 10 visits or 30 days, which can get you going on the right path faster and with fewer appointments. Direct access patients are still covered by most insurance companies (Workers Comp/No Fault and Medicare require a physician prescription). The therapists are always happy to speak with and include your primary care physician or physician of your choice to coordinate your care at any time. The therapists are also well equipped to determine if a referral to a physician is necessary to complete your care.
